As a CCBHC, Rosecrance is committed to facilitating access to a wide range of services for individuals in need.
Rosecrance Behavioral Health is proud to share its selection as one of the key contributors to the advancement of behavioral health services across Illinois. This achievement is a direct result of our successful application to the State of Illinois for inclusion in the Certified Community Behavioral Health Clinic (CCBHC) Medicaid Demonstration Program.
Rosecrance, as a grant recipient, has already been operating successful CCBHCs in two locations in the Rosecrance network – Rockford and Champaign. These CCBHCs will continue to operate under the new Medicaid demonstration project and strengthen mental health and substance use treatment through comprehensive community outpatient behavioral health treatment. These clinics play a vital role in providing a comprehensive range of care to individuals who require treatment, irrespective of their financial capability.
CCBHCs are mandated to deliver nine fundamental services: screening, assessment, and diagnosis; primary care screening and monitoring; crisis care for mental health; client-centered treatment planning; outpatient behavioral health services; case management; psychiatric services; peer and family support; and veterans’ services.
As a CCBHC, Rosecrance is committed to facilitating access to a wide range of services for individuals in need. This includes ensuring seamless coordination of care and integrating evidence-based practices to improve overall outcomes.
Moreover, Rosecrance will provide additional assistance tailored to meet the specific needs identified in a community assessment. This comprehensive approach encompasses the provision of 24/7 crisis services to ensure that individuals have access to immediate support and care when they need it most.
